This form is the FIRST form that must be submitted by a breeder. This registers you as a breeder, as well as your kennel name, and generates a permanent number that will identify you and animals produced from your kennel.

By submitting this form you are agreeing that all the information that you submitted on your kennel's behalf is truthful and accurate to you knowledge. In exchange you have permission to use the DDKC's initials and logo in relation to any animals registered or to be registered with this company.

Please note: Your Breeder Number will be provided via mail with the paperwork of your first litter registration. Please register your first litter without this number.
